Medical marijuana (cannabis) can be used to treat conditions like chronic pain, glaucoma, and cancer. To purchase medical cannabis from a dispensary, you must own a medical marijuana card (MMJ card). And to receive a card, you must complete and submit an application for the Michigan Medical Marijuana Program (MMMP) registry card.
An important step in finishing this application is to have a qualified medical marijuana doctor (mmj doctor) perform a medical marijuana evaluation. For those who want to gain the ability to purchase medical marijuana, Dr. Naeem can help.
During your appointment, Dr. Naeem will work with you to determine if medical marijuana is the right treatment option for you (part of this process includes reviewing your medical history). If eligible, you will leave the office with an important page in your application completed.
In Michigan, medical marijuana can be used to treat those with one or more of the following conditions:
Medical marijuana can also be used for one or more of the following symptoms of a debilitating or chronic disease, condition, or treatment:
You’ll need to bring the following to your medical marijuana evaluation:
We can give you the application paperwork while you’re in the office, or you can click this link to fill out the paperwork at home.
At your appointment, Dr. Naeem will review your medical history and discuss the current condition(s) being considered for medical cannabis. He will also let you know if any tests will be needed during this appointment to make or confirm a diagnosis.
Once eligibility is determined, Dr. Naeem will fill out, sign, date, and return to you the Physician Certification Form section of your MMMP registry card application.
Do not change anything on the Form or use white-out, as this will cause your application to be denied. It is your responsibility to complete and submit the form.
