      During and After Your Medical Marijuana Evaluation

      At your appointment, Dr. Naeem will review your medical history and discuss the current condition(s) being considered for medical cannabis. He will also let you know if any tests will be needed during this appointment to make or confirm a diagnosis.

      Once eligibility is determined, Dr. Naeem will fill out, sign, date, and return to you the Physician Certification Form section of your MMMP registry card application.

      Do not change anything on the Form or use white-out, as this will cause your application to be denied. It is your responsibility to complete and submit the form.
