And to receive a card, you must complete and submit an application for the Michigan Medical Marijuana Program (MMMP) registry card.

During your appointment, Dr. Naeem will work with you to determine if medical marijuana is the right treatment option for you (part of this process includes reviewing your medical history). If eligible, you will leave the office with an important page in your application completed.

At your appointment, Dr. Naeem will review your medical history and discuss the current condition(s) being considered for medical cannabis. He will also let you know if any tests will be needed during this appointment to make or confirm a diagnosis.
Once eligibility is determined, Dr. Naeem will fill out, sign, date, and return to you the Physician Certification Form section of your MMMP registry card application.
Do not change anything on the Form or use white-out, as this will cause your application to be denied. It is your responsibility to complete and submit the form.
